Included in the reportis a detailed failure mode analysis which quantitatively identifies criticalmodes of failure (both hardware and human) whose occurrence (singly or incombination) can cause hazard in flight. The identified failure modes are rankedin terms of criticality. Improvement recommendations are made and a method isdeveloped to assess their cost effectiveness.
